Participant Agreement/Waiver:
The undersigned participant agrees to abide by the policies / rules of the Fit Over Lunch Program (FOL). The undersigned participant understands that all use of the FOL services and programs shall be undertaken at his/her sole risk and the FOL program and the hosting company, shall not be liable for any injuries, illnesses, accidents, or deaths occurring to the participant, arising either directly or indirectly out of utilizing the FOL programs and services. Participant understands that the agreement to participate in the FOL Program is at their discretion and responsibility; and SWAT Fitness Club, the FOL instructors and the hosting company shall not be liable to participant for any claims, demands, injuries, damages, or actions arising due to injury to participant's person or property arising out of or in connection with the use by participant of the services, instructions and premises of the program. Participant hereby holds FOL, SWAT Fitness Club and the hosting company; their officers, owners, agents, instructors and employees harmless from all claims which may be brought against them by participant or person(s) representing participant for any and all such injuries or claims.
